Left Atrial Appendage Thrombosis Simulating Myxoma

چکیده
A 51-year-old man was admitted to the emergency department with palpitation and atypical chest pain. He had a history of percutaneous mitral valve commissurotomy 20 years previously. He did not use any drugs. Physical examination was unremarkable except for a loud S1 on auscultation. The surface electrocardiogram (ECG) showed atrial fibrillation rhythm with rapid ventricular response and right bundle branch block. Transthoracic and transesophageal echocardiographic examinations revealed normal left ventricular size with mild systolic dysfunction, ejection fraction of 45%, and moderate rheumatic mitral stenosis with mild to moderate mitral regurgitation. Also, there was a highly mobile multilobulated mass attached to the anterior wall of the left atrial appendage with a long stalk, simulating myxoma. The patient was referred for emergent surgery, during which the mass was removed. Pathological examination demonstrated organized thrombosis. Recently, Peters and et al.1 described a woman with significant mitral stenosis and a mass in the left atrium attached to the interatrial septum through a stalk; pathological examination showed thrombosis. In another report, a man with moderate mitral stenosis and a left atrial mass attached via a narrow stalk to the interatrial septum was described.2 In cardiac magnetic resonance imaging, thrombus often appears as a hypointense structure after the administration of intravenous gadolinium, whereas atrial myxomas show contrast enhancement. This imaging modality, however, is far from perfect.3

Myxoma is the most common type of primary tumors of heart. Recurrence of nonfamilial myxoma is unusual and recurrence in left atrial appendage is not reported. We present a case of sporadic myxoma in the left atrium who had a successful resection. However, the patient’s constitutional symptoms caused confusion and difficulty in diagnosis. Final diagnosis was recurrent Myxoma in the left atri...

A39-year-old woman was evaluated for an intra-atrial mass after having suffered a CT-verified stroke. She experienced a total remission of her neurological symptoms within 6 weeks. A search for embolic foci was conducted. Transthoracic echocardiography revealed a mass in the left atrium. MRI confirmed the diagnosis. At surgery, instead of the suspected myxoma, an inverted left atrial appendage ...

متن کاملLeft atrial appendage mass: is it always a thrombus?
Myxoma is the most common benign tumor of the heart, but it is very rare for it to originate from the left atrial appendage. Distinguishing between a mass, a thrombus, and a tumor in the body of the left atrium with preoperative transthoracic or transesophageal echocardiography is very difficult, even more so in patients with mitral valve disease and chronic atrial fibrillation. A 50-year-old m...

This study was conducted in 46 patients with cardiac thrombi, 15 patients with atrial myxomas, and 32 patients with other cardiac or paracardiac tumors. Diagnoses were subsequently proven by surgery, autopsy, computed tomography, magnetic resonance imaging, or angiography in all patients. All patients underwent precordial and transesophageal two-dimensional echocardiography to assess the variou...
